Assumption of Risk and Release of Liability

I, the undersigned parent or legal guardian of the above-named minor participant, give permission for the Participant to participate in baseball training, instruction, practices, clinics, camps, events, and related activities provided by Gold Glove Infielders, LLC.

I understand that participation in baseball and related athletic activities involves inherent risks, including but not limited to physical injury, illness, pain, suffering, disability, paralysis, emotional distress, property damage, or death. These risks may result from the actions or negligence of the Participant, other participants, coaches, instructors, spectators, equipment, field conditions, travel to and from activities, or other circumstances related to the Activity.

I acknowledge that I have considered these risks and voluntarily allow the Participant to participate. I understand and agree that participation is at the Participant's own risk, and I knowingly assume all risks, both known and unknown, associated with participation in the Activity.

Release and Waiver

In consideration for allowing the Participant to participate in activities conducted by Gold Glove Infielders, LLC, I, on behalf of myself and the Participant, hereby release, waive, discharge, and hold harmless Gold Glove Infielders, LLC, its owners, coaches, instructors, employees, volunteers, representatives, agents, cities, locations, facilities and affiliates from any and all claims, demands, causes of action, damages, losses, or liabilities arising out of or related to participation in the Activity, except where prohibited by law.

I understand that Gold Glove Infielders, LLC is not responsible for the acts, omissions, negligence, or failures to act of any third party, facility, organization, or individual involved with the Activity.

Medical Authorization

In the event the Participant requires medical attention, I authorize Gold Glove Infielders, LLC and its representatives to obtain and/or provide emergency medical care as deemed reasonably necessary, including first aid, CPR, AED use, emergency transportation, and communication with medical professionals.

I understand that I am responsible for all medical expenses incurred as a result of any injury or medical treatment provided. I acknowledge that the Participant should have appropriate health insurance coverage.

Participation Requirements

I understand that baseball activities may test the Participant's physical and mental abilities and may involve strenuous activity. I certify that the Participant is physically able to participate and has no known medical condition that would prevent safe participation unless disclosed to Gold Glove Infielders, LLC.

I agree that Gold Glove Infielders, LLC staff and representatives may remove the Participant from any activity if they determine, in their judgment, that participation presents a safety concern.

Media Release Authorization

I, the undersigned parent or legal guardian of the Participant, hereby grant Gold Glove Infielders, LLC ("GGI") permission to photograph, videotape, record, and otherwise capture the Participant's image, likeness, voice, name, and performance during training sessions, clinics, camps, events, and related activities.

I authorize GGI to use, reproduce, publish, distribute, and display such photographs, videos, recordings, and other media for lawful promotional, advertising, marketing, educational, and social media purposes, including but not limited to websites, social media platforms, printed materials, digital advertisements, and other promotional content, without further notice or compensation.

I understand that all photographs, videos, and recordings become the property of Gold Glove Infielders, LLC and may be used indefinitely unless prohibited by law. I release and hold harmless Gold Glove Infielders, LLC, its owners, coaches, employees, agents, and representatives from any claims arising out of the use of such media, including claims for invasion of privacy, defamation, or rights of publicity.

Term of Agreement

This Waiver and Release of Liability shall remain effective for the duration of the Participant's involvement with Gold Glove Infielders, LLC, including future activities, training sessions, camps, clinics, or events unless revoked in writing and confirmed by Gold Glove Infielders, LLC.
